FIRST! Please complete your pharmacy intern registration with the North Carolina Board of Pharmacy!!

Are you visiting from a pharmacy school in North Carolina? -You may have already completed this process. Please do so if you have not.

Are you visiting from a pharmacy school OUTSIDE of NORTH CAROLINA? Please ensure that you have completed intern registration with the NORTH CAROLINA Board of Pharmacy.
Please review the information on this page before your first day of rotation. Read over the syllabus and information for your first day. Complete the required applications, forms, and registrations (Dropbox) and let your preceptor know if you have any questions!

Syllabus & Grading Expectations

Information for Your First Day
Click here for information about directions and parking, rotation hours and dress code. You'll arrive at 10 AM on your first day for orientation.

CVM Visitor Application
*This form must be completed prior to your first day.*
Your CVM sponsor is Emily Sorah, and your visitor/volunteer affiliation is Pharmacy.

Rabies Vaccination Form
*This form must be completed prior to your first day.*
PLEASE NOTE: Rabies vaccination is NOT required to participate in this rotation. Select/Sign Line 3 if you have not already been rabies vaccinated.

Dropbox
Please create a Dropbox folder and share with Emily Sorah
(elsorah@ncsu.edu), Cassie Donnell (cedonnel@ncsu.edu) and Skyler Moore (sfmoore@ncsu.edu). This is how you will submit assignments.

Name your folder using your first initial and last name, followed by the month and year of your rotation.
Example: ESorah_AUG2017.

Please upload a copy of your CV (or resume) to your Dropbox folder prior to your first day.

Pre-Rotation Evaluation
Please complete the pre-rotation evaluation prior to your first day on site. This form will allow us to tailor the rotation experience to your goals and interest.
